Intake paperwork should be emailed to you prior to your first appointment. The email will be from Simple Practice, which is used for intake paperwork and scheduling.
Intake paperwork must be filled out at least 48 hours before your appointment or it may be cancelled.
Please be sure to bring your photo ID and a copy of your insurance card, if applicable.
